Managed Care Program
Accord Medical Group’s Managed Care Program follows a patient-centered model where our physicians dedicate more time to each patient, providing comprehensive, personalized care. Our mission is to improve the well-being of adults by coordinating essential health and wellness services to promote independence and overall prosperity.
To ensure the highest quality care, we partner with trusted specialists in the Tampa Bay area. For patients requiring off-site care, a dedicated staff member will manage the entire referral process, ensuring seamless coordination and support.
Enrollment & Eligibility
To qualify for the Managed Care Program, patients must:
- Be Medicare-qualified (Part A and B)
- Be enrolled in select Medicare Advantage plans
- Have Accord Medical Group or one of our physicians assigned as their Primary Care Provider (PCP)
